This Doctor Agreement (“Agreement”) is made between Medimyth (“Platform,” “we,” “us,” or “our”), operated by [Insert Company Name], and the Doctor (“you,” “your,” or “Doctor”) who registers to offer appointment slots through the Medimyth platform.
By registering and creating a schedule on Medimyth, you agree to the following terms:
1. Eligibility and Registration
1.1 You confirm that you are a licensed and practicing medical professional in India and hold a valid registration/license number issued by the appropriate Medical Council.
1.2 You agree to provide accurate personal and professional information, including but not limited to:
- Full Name
- Medical Registration Number
- Clinic Address and Contact Details
- Specialization and Experience
1.3 You understand and agree that Medimyth may verify your credentials from public or official sources or request documentation before approval.
2. Use of Platform
2.1 You may use the Medimyth platform solely for the purpose of:
- Creating your doctor profile
- Publishing your available schedule
- Accepting patient appointments
2.2 You are solely responsible for managing your availability and ensuring that scheduled appointments are honored unless cancelled in advance for valid reasons.
2.3 You agree that Medimyth is a facilitator only and does not provide any medical advice or services directly.
3. Platform Fee and Payments
3.1 Medimyth collects a non-refundable platform fee from patients at the time of appointment booking.
3.2 You understand that Medimyth does not collect consultation fees on your behalf, and any such transactions must occur directly between you and the patient.
4. Communication and Patient Privacy
4.1 You will only receive limited patient data (name, contact number, appointment time) after an appointment is booked.
4.2 You agree to maintain strict confidentiality and comply with all applicable medical ethics and privacy laws.
5. Code of Conduct and Compliance
5.1 You agree to:
- Maintain professionalism and punctuality
- Avoid any unethical or misleading medical claims
- Not misuse the platform for illegal or fraudulent activities
5.2 Medimyth reserves the right to suspend or permanently remove your profile from the platform in the event of:
- Misrepresentation of credentials
- Patient complaints or professional misconduct
- Violation of any terms herein or relevant medical laws
6. Indemnity and Liability
6.1 You agree that all medical advice, diagnosis, and treatment offered to patients are your sole responsibility.
6.2 Medimyth shall not be held liable for any claim, loss, or dispute arising from medical services rendered by you.
6.3 You indemnify and hold harmless Medimyth from any legal action or damages arising from your use of the platform or your conduct as a medical practitioner.
7. Term and Termination
7.1 This Agreement shall remain valid as long as you continue using the Medimyth platform.
7.2 Either party may terminate this Agreement by giving written notice.
7.3 Medimyth reserves the right to suspend or terminate your account immediately in case of:
- Breach of terms
- Legal issues or regulatory action
- Verified misconduct or complaints
8. Governing Law
This Agreement shall be governed by and construed under the laws of India, and any dispute arising out of this Agreement shall be subject to the exclusive jurisdiction of the courts in [Insert City], India.
